What is the Community Living Alliance CIP/COP?

The CIP and COP Programs are two of several state and federally funded Long Term Support Programs which offer information and financial assistance to people with physical disabilities and long term illnesses. These programs can be accessed through the Community Living Alliance (CLA). The goal of the programs is to provide community based services to support and promote consumers' independence in the community.

Who is eligible?

Eligibility is determined by the following criteria. Applicants must:

  • Be between the ages of 18-64.
  • Be a resident of Dane County.
  • Have a long term irreversible illness or physical disability which substantially impairs function.
  • Meet financial eligibility based on income and assets.

What benefits are provided?

Long Term Support programs offer information about and funding for community based needs. Some of the services funded include:

  • Housekeeping and chore services.
  • Laundry/shopping/personal cares services, such as assistance with bathing and dressing.
  • Meal preparation or delivery.
  • Adaptive equipment.
  • Emergency response systems.
  • Service coordination.
  • Transportation.
  • Adult family home and group home care.
  • Attendant care locator services.

Choice in services

The goal of the CLA CIP/COP program is for consumers to have a choice in deciding which services are needed and which vendors to use. Decisions regarding the consumer's service plan are made jointly with the consumer and service coordinator. In all aspects of the program, the preferences of the consumer are respected.
